Walk-in || Medial Social Worker

• Will be responsible to receive the patients and to ensure that registration is done for new patients.
• MSW will be responsible for collecting demographic details of the patients through the Registration form.
• The MSW will also have to capture details of the referring doctor and patients family doctor details.
• Provide psychosocial assessment and counselling support to patients and families.
• Assist patients in addressing social, emotional, financial, and family-related concerns.
• Coordinate with psychiatrists, psychologists, nurses, and other healthcare professionals.
• Conduct patient and family interviews and maintain appropriate case records.
• Support treatment adherence, rehabilitation, and community reintegration.
• Identify social support systems and connect patients with relevant welfare schemes and community resources.
• Assist with discharge planning and appropriate referrals.

Preferred: Specialization or experience in Medical & Psychiatric Social Work / Mental Health.
