Walk-in || Clinical Pharmacist

Role & responsibilities

• Review prescriptions for dosage accuracy, drug interactions, contraindications, and medication safety.
• Participate in clinical rounds and provide evidence-based recommendations for optimising drug therapy.
• Conduct medication reconciliation during patient admission, transfer, and discharge.
• Counsel patients and caregivers on medication usage, adherence, side effects, and precautions.
• Monitor and report adverse drug reactions (ADRs), medication errors, and drug-related problems.
• Collaborate with doctors, nurses, and other healthcare professionals to ensure rational and safe medication use.
• Maintain clinical documentation and ensure compliance with hospital protocols and accreditation standards.
• Participate in medication audits, pharmacovigilance, antimicrobial stewardship, and quality improvement initiatives.

Preferred candidate profile

Pharm D

• Candidates with hospital-based clinical pharmacy experience or completed clinical internships.
• Strong knowledge of medication safety, prescription review, drug interactions, and patient counselling.
• Candidates with exposure to clinical rounds, medication reconciliation, and pharmacovigilance will be preferred.
