Job Summary

1. Clinical Associate to assess patients vitals and history and document

the same in proper order.

2. Taking clinical rounds of patient and accompany primary and referring

consultant on their rounds and convey the orders to assigned staff and

keep track on the same.

3. Clinical Associate to channelize for discharge of patient one told fit for

discharge by consultant.

Job Profile

1. Receive the patient, examine her, writing history and findings in the indoor case sheet. To take patients &/or relatives Signature for authentication of information provided and noted.

2. Inform and co-ordinate with primary consultants regarding patient admission and plan of care.

3. Decide investigation to be done and treatment to be carried out

4. To follow up on investigation reports and communicate the results to primary consultants.

5. To take appropriate actions in emergency situations and treat the patient in consultation with the primary consultant.

6. To take detailed daily rounds at the time of reporting and leaving duty, to write relevant dates and time in progress note.

7. Follow all medico legal formalities.

8. To work in coordinator with staff nurse and colleague doctors to perform detailed handing over of the patient to the next duty doctor.

9. To counsel and inform patients condition to patient &/or relatives as and when required.

10. Prepare the patients for procedure and operations

11. Explain the procedure, alternatives and the associated risk to patients and her relatives.

12. Coordinate the discharge of the patients with primary consultants and nursing staff.

13. Prepare discharge summary and explain the same to patients.

Experience

1-5 years of clinical experience in a reputed institution preferably with Obstetrics experience

Job Profile

1. To receive patient, examining them, writing history and finding in the case sheet.

• To take patients & / or relatives’ signature on the same

2. To inform NICU/Primary consultant & decide the investigations & treatments to be done.

3. To follow up of investigation reports & communicate to the NICU/Primary Consultant

4. To inform any new development in the patient and plan the treatment consultation with the NICU/Primary consultant and note down the same in the progress notes.

5. To take appropriate action in emergency situations and treat the patient in Consultation with the clinical association, and to keep the NICU / Primary consultant informed.

6. To prepare the parents for procedures & operations.

7. To perform procedures after consulting the Consultant informed.

8. To take detailed daily rounds on reporting & leaving duty, to write relevant notes with date & time in progress notes.

9. To inform all referrals to consultants, communicate their advice to primary / consultant / Intensivist & follow their advice for investigation & treatment.

10. To complete all relevant entry and data in the patient file.

11. Follow all Medico-legal formalities.

12. To work in coordination with staff nurse & colleague doctors

13. To perform detailed handling over of the patient to the next duty doctor.

14. To attend all CME activities in the department and hospital

15. To inform the patient's conditions to the relatives when required.
