This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Sr. Patient Care Integrator based in United States.
This is a remote healthcare role focused on coordinating complex, patient-centered care from initial request through follow-up. You will serve as a trusted central point of contact, connecting patients with physicians, specialists, and care teams. The role combines care coordination, clinical documentation, patient communication, and personalized support. You will help ensure medical information, testing, procedures, and treatment plans are organized and accessible. Your work will directly contribute to a seamless healthcare experience built around each patient's individual needs. The position suits an experienced healthcare professional who is highly organized, empathetic, and comfortable managing complex situations. After completing onboarding, you will also participate in a patient-centered on-call rotation providing 24/7 support.
Accountabilities:: Coordinate and oversee patient care requests, ensuring timely, compassionate responses and prioritizing patient needs and well-being.
Serve as the central point of contact throughout the patient journey, coordinating communication among patients, providers, specialists, and care teams.
Maintain accurate, complete, and current electronic medical records (EMRs) so that relevant information is readily available to care providers.
Coordinate medical testing, procedures, referrals, and follow-up care while ensuring patients understand their care plans and next steps.
Prepare personalized care summaries and medical briefs that clearly communicate treatment pathways and help patients feel informed and supported.
Oversee the customization and distribution of Personal Medical Kits according to individual patient medical needs.
Participate in the patient-centered on-call rotation, providing 24/7 patient support within 90 days of employment.
Support the care team with unexpected or complex situations through calm, effective problem-solving and coordinated action.
Ensure patient interactions and care coordination activities comply with applicable healthcare regulations, including HIPAA and confidentiality requirements.
Contribute to additional patient-care initiatives and continuously support a compassionate, patient-first care environment.
Requirements:
Associate degree in nursing, social work, or a related healthcare field with a strong focus on patient-centered care; an RN qualification is preferred.
At least 5 years of healthcare experience, including demonstrated experience in patient care coordination and leadership.
Proven ability to coordinate and integrate complex care plans while maintaining high standards of quality and continuity.
Strong organizational skills, attention to detail, self-direction, and the ability to manage multiple patient needs effectively.
Excellent written and verbal communication skills, with the ability to communicate compassionately with patients, families, and healthcare professionals.
Strong creative problem-solving abilities and a commitment to improving the patient experience and care delivery.
High integrity and a strong commitment to protecting confidential patient health information.
Proficiency with Microsoft Office, Excel, and electronic health record systems.
Ability to work with computers and medical or office equipment, review detailed clinical information, and remain focused during extended periods of computer-based work.
Ability to lift up to 25 pounds and sit for extended periods as required by the role.
Willingness to travel overnight for training or meetings and possession of a valid driver's license in good standing.
Ability to work effectively in a remote environment while maintaining reliable communication, organization, and patient-service standards.
Benefits:
Salary: $68,000–$73,000 USD annually.
Work arrangement: Fully remote within the United States.
