Role & responsibilities

• Verify patient demographics, insurance eligibility, provider details, CPT, ICD, and modifier information.
• Check the claim status through insurance portals or by contacting the payer.
• Contact insurance companies to understand the exact reason for the denial.
• Obtain and document the claim number, reference number, denial reason, and next action.
• Identify common denials such as:
• Eligibility and coverage issues
• Authorization or referral required
• Denials
• Hospital billing
