Hcc Medical Coder

Role & responsibilities

• Review patient medical records, including progress notes, physician documentation, and discharge summaries.
• Identify and validate diagnoses documented by healthcare providers.
• Assign accurate ICD-10-CM codes according to official coding guidelines.
• Identify applicable Hierarchical Condition Categories (HCCs) for risk adjustment.

Preferred candidate profile

• Graduate in Life Sciences, Nursing, Pharmacy, Allied Health Sciences, or another relevant medical field.
• Knowledge of medical terminology, human anatomy, physiology, and disease processes.
• Basic to good knowledge of ICD-10-CM coding guidelines and HCC risk adjustment.
