HCC Medical Coder

Job Summary

Minimum 1 year of relevant HCC / Risk Adjustment coding

experience.

• Active CPC (Certified Professional Coder) certification is

mandatory.

• Must provide valid CPC certification details for verification.
• Must not be currently employed / actively working with any

other organization at time of joining.

• Candidates serving notice period may be considered, subject

to company policy.

• Strong knowledge of ICD-10-CM, HCC/Risk Adjustment

concepts, medical terminology, anatomy and clinical

documentation.

• Experience reviewing medical records and identifying

reportable diagnoses preferred.

• Good coding accuracy and attention to detail.
• Ability to meet productivity and quality targets.

Active CPC Mandatory

KEY RESPONSIBILITIES

• Review medical records and clinical documentation for

HCC/Risk Adjustment coding.

• Identify and accurately assign appropriate ICD-10-CM

diagnosis codes.

• Apply HCC and Risk Adjustment coding guidelines.
• Ensure diagnoses are supported by appropriate clinical

documentation.

• Identify relevant chronic conditions and comorbidities

from patient records.

• Maintain required coding accuracy and productivity

standards.

• Follow client-specific coding guidelines and compliance

requirements.

• Participate in quality audits; address coding errors or

feedback.

• Maintain confidentiality of patient and client information
