Geriatric Coordinator

Responsibilities

• Coordinate and follow up with geriatric consultants regarding patient treatment, follow-up appointments, and ongoing care requirements.
• Build and maintain professional relationships with geriatric consultants and facilitate appropriate home-care referrals.
• Coordinate with the Geriatric Department to understand patients' clinical conditions and home-care requirements.
• Attend calls and enquiries related to geriatric home-care services and provide appropriate information to patients and caregivers.
• Understand patient and caregiver requirements and coordinate with the clinical team to identify suitable home-care services.
• Counsel patients and caregivers regarding available geriatric home-care services, packages, care options, and service benefits and explain home-care packages, inclusions, duration, pricing, terms, and service conditions.
• Coordinate the end-to-end home-care process from receiving the requirement to service monitoring and closure.
• Coordinate with the Nursing Department to allocate geriatric nurses based on patient care requirements.
• Provide orientation to assigned nurses regarding the work and applicable service protocols.
• Monitor continuity of home-care services, including nurse attendance, scheduled visits, and service requirements.
• Coordinate nurse replacements in cases of leave, absenteeism, performance-related concerns, or patient requests.
• Maintain regular communication with patients, caregivers, and assigned nurses throughout the service period.
• Address routine patient and caregiver queries and coordinate with concerned departments for timely resolution.
• Obtain patient and caregiver feedback and escalate complaints or service-related concerns to the appropriate authority.
• Coordinate with the Billing/Accounts Department regarding package confirmation, payment status, renewals, extensions, and other service-related requirements.
• Maintain effective coordination with the Nursing Department, Geriatric Department, Billing/Accounts Department, and other relevant hospital departments.
• Serve as a coordination link between geriatric consultants, patients, caregivers, nurses, and internal departments to ensure seamless service delivery.
• Maintain patient confidentiality and ensure compliance with hospital policies, procedures, and service protocols.
• Promote awareness of geriatric home-care services among consultants, patients, caregivers, and relevant stakeholders.
• Participate in periodic review meetings and provide updates on referrals, conversions, active cases, renewals, service concerns, and operational challenges.
